ACKNOWLEDGEMENT OF RISK AND RELEASE OF LIABILITY I am aware that my organizations/teams' usage of Legion and LaFortune Fields exposes the applicant to risk of physical injury by the nature of the sport. This being considered, I do release and exonerate Tulsa Little League, Inc., Tulsa County, the Board of County Commissioners, their agents and employees from any and all liability as a result of injury associated with participation in this sports activity. I, the applicant will abide by the rules of Tulsa Little League, Inc. for usage of the field. I do release and exonerate Tulsa Little League Baseball, Inc. its Board of Directors and associated personnel from any and all liability as a result of the our participation in the program or being transported to or from the same, which transportation I hereby authorize. 2. I acknowledge that all fees are non-refundable.
